The inquest, led by Coroner Emma Mather, found that Tommie-lee's death was a result of "misadventure" rather than a deliberate game or prank.

During the sleepover on March 1, a group chat was created on Snapchat, where a child noted that Tommie-lee and his friend were inhaling toxic substances during a video call. According to witness accounts, Tommie-lee appeared pale and unwell, but was encouraged by others in the chat to continue. At that point, he expressed reluctance, indicating a lack of awareness of the serious dangers posed by such actions.

Upon returning home after an activity with another child, Tommie-lee's mother noticed a strong smell emanating from the bedroom. When she entered, she rolled her son over and observed a "tinge of blue," prompting her to call for an ambulance. Sadly, it was too late.

The investigation highlighted that Tommie-lee and his friend were reportedly "doing it for a laugh," according to a statement from Detective Chief Inspector Dylan Hrynkow of Lancashire Police. However, thorough searches of the mobile phones belonging to both boys showed no internet searches or communications about social media challenges that could have led to the incident. Instead, it was suggested that another child at a skate park had introduced them to this dangerous behavior.

Compounding the tragedy, the coroner learned that three months prior, Tommie-lee and his friend had been caught inhaling toxic substances in a similar setting. The mother of Tommie-lee's friend claimed she informed the school, the police, and Tommie-lee's parents about that prior incident, although Tommie-lee’s parents disputed having been informed.

Coroner Mather pointed out the possible lack of supervision at the sleepover, a sentiment echoed by Detective Hrynkow, who noted signs of inadequately monitored activities. Despite these concerns, the Crown Prosecution Service concluded that there was insufficient evidence to pursue criminal charges against anyone involved.
